From Georgia Ghost Hunters Case Reports

There are many forms we use for each investigation. It is necessary to document everything form start to finish to assure you have all the needed information to write up your case file. I will explain briefly what forms we have and what they are used for. We have alot of these pre-made forms available in our downloads section of the main website.

• Preliminary Form
  This is the form that clients fill out explaining everything about the location, how many people are involved with the phenomena, the strange occurences that they have witnessed, plus many many more questions that are vital for any team to know about before setting up the formal investigation.
• Paranormal Investigation Permission/Release Form
  This form is used to have the proper permission to be at said location. The client signs and returns this form to you so you will have proof that you are allowed to be at the location. It also states that the client will not be responsible for any harm that may come to the team and protects them form lawsuits. In addition, the investigators assume responsibility for any damages to the property that may take place during the investigation.
• Spiritual/Demonic Attack Waiver Form
  This is a new form we have just started using, I would recommend teams thinking about using a form of this nature. This form is used to protect the team, affiliates and clergy, stating that we are not responsible for any physical or psychological attacks by spiritual or demonic entities, during or after any investigation.
• Confidentiality & Release Of Information Form
  This form is used to ask the client to check off what level of privacy they request.
• Temperature Reading Form
  We use this form to jot down base readings before the investigation so if we get a certain reading during the investigation we will have this to compare any abnormal readings.
• EMF Reading Form
  This is used to gather baseline readings at the location so if we have any spikes out of the ordinary we can compare to the baseline reading form.
• Phenomena Witnessed Form
  This is a must to use. We use this to document the time, room and explanation of what was witnessed or happened during investigating. Each team must have one of these including Head Quarters to write anything out of the ordinary down. This also helps with reviewing evidence in case something manmade was heard then any audio that sounded out of place can be compared to this form.
